Job Title: Targeted Adults Support Worker

Locations and Hours: 18 hours per week or 35 hours per week (Permanent). Location: Wildwood, Worcester. Team: Targeted Adults Support Team.
Salary: £13,913 to £15,092 per annum (based on 18 hours); £27,052 to £29,345 per annum (based on 35 hours).

The Role
This is a great opportunity to be part of an exciting transformation of the Adult Front Door service. We support people at the earliest opportunity and maximise the effectiveness of early intervention and prevention. The Adult Front Door Service is the first point of contact for individuals seeking universal health and wellbeing support. The successful candidates will provide a person-centred response to requests for help and effectively support individuals to live well and as independently as possible. They will also act as a lead professional to develop and sustain collaborative links with a range of agencies and practitioners in order to co-ordinate and deliver bespoke support packages.
As a Targeted Adults Support Worker, you will work with individuals using a strength-based approach, who may have a wide range of short-term care and support needs and require support to reduce and manage identified risks. You will offer professional support, practical advice and guidance to individuals on issues such as welfare benefits, budgeting, tenancies, life skills, healthy eating, lifestyles, education, training, and employment and draw up and deliver an agreed plan of intervention which supports improved outcomes.
