Stoke-on-Trent City Council Our service is a busy, supportive environment made up of three integrated teams: Home First, the Integrated Discharge Hub, and the Community Bed Team. We are co‑located to provide a strong multidisciplinary approach, ensuring the best possible outcomes for the people we support.
Key Responsibilities
- Undertake person‑centred wellbeing assessments
- Work collaboratively with health professionals and partner agencies
- Support timely hospital discharges and reduce delays
- Manage a varied caseload within a 7‑day service
