Stoke-on-Trent City Council – Adult Social Care & All Age Commissioning

Grade 10, 37 hours per week, permanent or fixed-term (6 months/12 months) for South East Wellbeing Team or North Wellbeing Team.

Location: South East Wellbeing Team / Stoke North Children’s Centre.

The Role

We are seeking a motivated, compassionate, and committed Social Worker to join our Adult Social Care Wellbeing Teams. The focus is on early intervention, prevention, and enabling people to live fulfilling and independent lives. The team uses a strengths-based approach, works creatively with individuals and partner agencies across health, housing and the voluntary sector, and delivers person-centred interventions that are outcome-focused and proportionate.

Key Responsibilities

  • Undertake strengths-based assessments and support planning in line with the Care Act 2014
  • Work collaboratively with individuals to identify goals and build on existing strengths and networks
  • Promote independence, early intervention and prevention
  • Sign post and connect individuals to community resources and services
  • Manage a caseload proportionate to your experience and work in partnership with health professionals, voluntary sector organisations and internal teams
  • Participate in safeguarding enquiries where required and maintain accurate and timely case records

About You

  • Qualified Social Worker registered with Social Work England
  • Knowledge of relevant legislation, including the Care Act 2014 and the safeguarding framework
  • Strong commitment to strengths-based and person-centred practice
  • Excellent communication and partnership working skills; able to manage competing priorities and work effectively in a fast-paced environment
  • Shows initiative, resilience and a proactive approach
