The Role
Are you a passionate and forward-thinking Occupational Therapist looking for an opportunity to make a meaningful difference? We are seeking an enthusiastic Occupational Therapist to join our innovative Step Up/Step Down Project Team on a 12-month fixed-term contract. This exciting role will support individuals accessing a Temporary Night Support Service and/or 24-Hour Wrap Around Support Service, helping them to remain safely and independently in their own homes. This is a unique opportunity to be part of a service that provides timely, intensive support to prevent unnecessary hospital admissions, facilitate safe discharges, and reduce reliance on long-term care.
Key Responsibilities
- Assessing functional abilities and independence with activities of daily living, such as personal care, mobility, meal preparation, and medication management.
- Identifying risks within the home environment and recommending equipment, adaptations, or community support services to mitigate those risks.
- Supporting rapid discharge pathways by carrying out timely assessments that would otherwise delay patient discharge.
- Providing advice and education to patients, families, and carers to maximise independence and confidence at home.
- Collaborating with health and social care professionals to develop holistic, person-centred discharge plans.
- Facilitating access to community rehabilitation services and reablement support where appropriate.
- Contributing to admission avoidance by assessing patients at risk of hospitalisation and implementing alternative support arrangements where feasible.
