Staffordshire County Council – First Contact Service – Social Care Assessor. The service delivers a single point of access for all-age carers. Assessments are undertaken in person across the county. The role also supports Assistive Technology and the Quick Fix Fund.
Main Responsibilities
- Take responsibility for the provision of assessments under the Care Act 2014 inclusive of all statutory functions.
- Develop a collaborative relationship with adults and their circles of support to promote positive expectations, understanding talents, wishes, and feelings.
- Be part of the assessment process, undertake purposeful interaction with individuals, families and other professionals to understand needs and strengths.
- Critically analyse information to understand risks and develop strategies to explain, avoid or manage risk appropriately.
- Negotiate and agree appropriate, tangible and realistic outcomes with individuals and families, identifying strengths.
- Collaborate with other professionals within Adult Social Care or other organisations to enable individuals to achieve their outcomes and independence.
- Maintain appropriate professional records of all key discussions, findings, assessments, opinions and decisions, including electronic client records and performance data, in line with policies and procedures.
The Ideal Candidate
- Assessment and risk assessing skills.
- Ability to work under pressure.
- Knowledge and understanding of the Care Act 2014 and the Mental Capacity Act 2005.
- Understanding of disability, older persons, carers and young carers and the impact for individuals, families and society.
- Knowledge of adult physical, intellectual, emotional and social development and family dynamics.
