Overview

An exciting and rewarding opportunity has arisen for the recruitment of a Social Worker to join our Physical Disability & Sensory Service Team (PDSS) within Warwickshire. Warwickshire County Council is a top performing two tier authority with a reputation for innovation and efficiency. Our team operates county wide and has bases in Warwick and Bedworth and comprises Social Care Practitioners and Social Workers. We are a dedicated and supportive team that aims to respond quickly and efficiently by offering strengths-based assessments and providing support to individuals and their carers within a fast-paced environment.

We work closely with health colleagues offering ample multidisciplinary experience. The team works with a mix of office based and remote working. You will be provided with monthly supervision alongside monthly team meetings and CPD sessions.

Responsibilities

  • Provide high-quality professional assessment, support planning and review services to the people we support.
  • Work with individuals who require a safeguarding response or are at risk due to self-neglect.
  • Complete a significant number of Mental Capacity assessments around key life decisions, in line with the Care Act.
  • Undertake outcome-focused assessments, support planning and reviews; carry out duty work as required.
  • Utilise a strengths-based, asset-based approach to enable customers to identify outcomes, maximise independence and reach their full potential.
  • Contribute to the development and promotion of the PDSS Team; work in imaginative, creative and innovative ways to meet support needs.

Qualifications and Requirements

  • Full clean Driver’s Licence
  • Social Work Qualification which includes Diploma/Degree/Masters
  • Social Work England Registration
  • Ability to work closely with the Multidisciplinary Health & Social Care team
  • Participate fully in the duties of the team, including team meetings and other meetings determined by the Team Leader and/or Operations Manager
  • A dynamic, strength-based approach
  • Ability to make difficult decisions and work to tight deadlines
  • Experience in adult social care and good Information Technology (IT) skills
  • In Warwickshire, use of the Mosaic IT System
