Do you fancy working in the Discharge to Assess team, where you would work with health and social care colleagues and care homes, attending Continuing Health Care meetings and planning discharge from Discharge to Assess beds?

We are a small county-wide team, and you can work from different offices like Shire Hall, Warwick or Kings House, Bedworth. We are a dedicated and supportive team. We aim to respond quickly and efficiently in offering strengths-based assessments.
