Herefordshire Council is recruiting two Social Care Assessors to provide high-quality assessments and support planning for vulnerable adults in Herefordshire.
The roles are 37 hours per week, permanent, based in Ross-on-Wye with a flexible hybrid working arrangement.
You will carry out person-centred assessments, create support plans, conduct reviews and risk assessments, and work with families, carers and partner agencies to maximise independence and wellbeing.
