Sandwell Metropolitan Borough Council is looking to appoint a STAR Trusted Assessor. 18.5 hours per week.

Responsibilities

  • Carry out comprehensive assessments in hospital and community settings across the Borough of Sandwell.
  • Identify individual needs and personal outcomes through thorough assessment.
  • Develop personalised support plans aimed at meeting identified needs.
  • Complete appropriate risk assessments to identify potential risks.
  • Implement actions and strategies to minimise identified risks.

Essential Requirements

  • Minimum of 1 year’s experience working with vulnerable adults and carers.
  • Knowledge of assessment frameworks, including risk assessment.
  • Ability to travel throughout the borough as required.
  • Availability for 7-day working, including some unsocial hours.
  • GCSE English and Maths (or equivalent qualification).
