Warwickshire County Council is recruiting two full‑time Reablement Officers (37 hours a week). The post is based at Heathcote House, Leamington Spa but covers the whole county. The role requires a rota of late shifts (12pm – 8pm) and early shifts (9am – 5pm), one weekend in seven, and bank holidays. Detail will be discussed at interview.

Key Responsibilities

  • Work with people in their own homes to help them achieve independence with daily living tasks.
  • Complete initial assessments, risk assessments, and identify which daily living tasks can be re‑abled.
  • Create a support plan that Reablement Assistants will use to help the person achieve their outcomes, and review and revise the plan at agreed intervals.
  • Assess and prescribe items of equipment to enable independence and reduce the need for ongoing support.
  • Ensure the person has gained a suitable level of independence that is maintained long‑term.
  • Liaise with colleagues, service users, health professionals and the public face‑to‑face or by telephone, displaying courteous, professional behaviour.
  • Work independently as well as part of a team, driving within the county to support the service.
  • Maintain confidentiality of information and adhere to Warwickshire County Council Policies and Procedures.
  • Commit to anti‑discriminatory practice in employment and service provision.
