Job Title: Reablement Co-Ordinator

Location: Wildwood, Worcester • Team: Reablement • Hours: 37 per week • Salary: £26,403 – £28,142 per annum

Contract: Permanent & Fixed Term Contracts Available

About the Service

The Reablement Service provides support to individuals who live in their own home. The service is registered with the Care Quality Commission and is based at Wildwood, County Hall Campus. This service is provided across the whole of the county of Worcestershire.

The service focuses on short term packages of care aimed at maximising people’s independence (Reablement). Clients are referred to the service by hospitals to support discharges (Pathway 1) and by Community Reablement Teams.

Responsibilities

  • As a co-ordinator you will be responsible for creating client records on our rostering system, creating & allocating visits to appropriate care workers, updating care & visit plans as the needs of the client changes, maintaining workers rota patterns, leave requests and training records.
  • You will be liaising with care workers daily to ensure that all client visits are allocated so excellent telephone manner and negotiation skills are essential for the successful applicant.
  • Ensuring that all clients receive a visit when due is integral to the role and good organisational and prioritisation skills are essential to the role as well as attention to detail. You will need excellent communication & recording skills as well as good problem solving and the ability to think outside the box.
  • The service is fast paced and changes to client’s need are constant so the ability to react positively and pro-actively is crucial.
  • A good knowledge of Worcestershire is important.
  • The Reablement service operates 24 hours per day everyday of the year including Bank Holidays. The office at Wildwood is manned from 7:00 to 22:00 daily. As a co-ordinator you would work on a 8 week rota between these hours and it would include working 2 weekends. The nature of the service however does mean that flexibility in working patterns is sometimes needed.
