This is a great opportunity for the right person to be part of an exciting transformation of the Adult Front Door service. We support people at the earliest opportunity and maximise the effectiveness of early intervention and prevention. The Adult Front Door Service is the first point of contact for individuals seeking universal health and wellbeing support.
The successful candidate will provide a person‑centred response to requests for help and effectively support individuals to live well and as independently as possible. They will also act as a lead professional to develop and sustain collaborative links with a range of agencies and practitioners in order to co‑ordinate and deliver bespoke support packages.
As a Targeted Adults Support Worker, you will work with individuals using a strength‑based approach, who may have a wide range of short‑term care and support needs and require support to reduce and manage identified risks. You will offer professional support, practical advice and guidance to individuals on issues such as welfare benefits, budgeting, tenancies, life skills, healthy eating, lifestyles, education, training, and employment and draw up and deliver an agreed plan of intervention which supports improved outcomes.
