Worcestershire County Council is recruiting a Social Care Worker for the Community Hospitals/Urgent Care teams based at Princess of Wales Hospital, Bromsgrove. This is a permanent, full-time role with a workload spread across county wards and opportunities to develop within the Discharge to Assess model.
You will collaborate with the Social Work Team and NHS colleagues, maintain a case load with support from a Qualified Social Worker for complex cases, and participate in MDT meetings.
