Dudley Metropolitan Borough Council’s Access & Prevention team within Adult Social Care supports people at the first point of contact in a call‑centre environment. As a Social Care Advisor you will focus on timely interventions to maximise independence and wellbeing, interpreting callers’ needs and initiating appropriate service delivery. You will screen for the Integrated Falls Pathway, Carers Network and other preventative services.

Hours: 37 per week (full time permanent) Salary: Grade 5 – £26,403 per annum

Responsibilities include:

  • Screening clients or their representatives at the first point of contact for new and ongoing referrals for adult social care via telephone, email or online forms.
  • Using IT and phone systems to gather client information and resolve enquiries where possible, advising callers of available services and eligibility criteria.
  • Triaging and referring to appropriate services; signposting to external or internal agencies, acting as Community Information Champions.
  • Collaborating with multidisciplinary teams (Occupational Therapy, Social Work, Care Assessment, MASH, Disability Service, Emergency Services and NHS partners) to ensure consistent, outcome‑focused support.
  • Contributing to the development, performance, quality assurance and continuous improvement of Adult and Integrated Care, supporting the Council’s objectives to safeguard adults, promote health, independence and wellbeing.
  • Maintaining up‑to‑date knowledge of community services and liaising with colleagues and partner organisations to access information and services.

Workplace: office 3+ days a week (pro‑rate) or onsite full time. Dudley Metropolitan Borough Council is committed to diversity and inclusion.
