Working 36.5 hours per week

We are welcoming applications for the Specialist Practitioner Social Worker role within our newly established Priority Discharge Team.

Responsibilities

  • Provide support to patients with specific health needs, their carers and the multi‐disciplinary team through the relevant patient pathway.
  • Ensure the effective management, operation and development of Adult Social Care to achieve relevant Trust and City Council objectives and statutory duties.
  • Participate in the identification, systematic assessment and review of service needs within the specialist area of responsibility and plan and deliver appropriate adult care services, exploring innovative approaches to service delivery.
  • Provide the particular social work skills and knowledge specific to the specialist area of responsibility.
  • Assess and review patients changing needs, providing continuity of care in a proactive and fluid manner.
  • Work autonomously with the specialist team, in partnership with the multi‐disciplinary team, to ensure social aspects of care are promoted and addressed.
  • Act as specialist adviser to other social care and health professionals and other agencies as required.

Qualifications & Experience

Applicants should possess relevant social work qualifications and experience, with a strong record in adult social care and specialist practice.

Requirements

  • A Disclosure and Barring Service (DBS) check will be undertaken.
  • Proof of Right to work in the UK will be required.
