About Us

Warwickshire County Council is looking for a full‑time Reablement Co‑ordinator to join our Reablement team of Occupational Therapists, Occupational Therapy Assistants, Reablement Officers, Supervisors and Assistants, and Social Care Practitioners. The role supports customers to remain in their own homes and achieve independence, preventing hospital or residential admissions.

The Role

As Co‑ordinator you will line‑manage Reablement Assistants, work with the Assessment Team and other professionals to deliver an outcome‑focused service that promotes independence and maximises daily living skills. You will develop and manage work programmes, manage referrals through the adult journey, and ensure plan outcomes are met. The post is 37 hours per week, based at Kings House, Bedworth, with countywide support duties requiring travel across Warwickshire. Shift pattern is 7 am‑3 pm, 9 am‑5 pm and 2 pm‑10 pm Monday‑Friday, with one weekend every three weeks and bank‑holiday rota.

Key Requirements

  • Dedicated individuals who can manage time, communicate effectively, work collaboratively or independently, handle confidential information, and travel across the county.
  • Flexible, solutions‑focused and experienced in a fast‑paced social care environment.
  • Experience managing a small team, and knowledge of Reablement practice is desirable.
