Care Navigator – 36 hours per week x 2 posts. 1 x post for South Community Team currently based in Pennant House, Bebington. 1 x post for Clatterbridge Intermediate Care Centre (CICC) - This role is currently split between office bases located at Pennant House, with additional in-reach support to the Clatterbridge Hospital Reablement Wards.
We are looking to appoint a Care Navigator who will play a key role in supporting the delivery of personalisation and inclusion pathways for adults and carers, ensuring compliance with the legal framework of the Care Act 2014. You will ensure strength‑ and asset‑based practice, personalisation and social inclusion are embedded and applied effectively throughout your work. The role will involve assessing and implementing a range of social care services and providing guidance, advice and information for adults, older people and carers. You will work autonomously and independently, assessing, formulating, implementing and agreeing care and support plans with adults and carers. You will be responsible for supporting the delivery of health and social care functions for adults and carers across Wirral. You will work in partnership with other agencies, supporting adults and carers to access services and support within the independent and non‑statutory sectors.

About the Candidate: We are looking for someone who can be part of an integrated and supportive team, planning, monitoring and evaluating work alongside the MDT care team. You will have experience working in Hospital Discharge and supporting adults, carers and families, helping to promote self‑determination, choice and autonomy. You will have knowledge and understanding of the Care Act (2014), Equality Act (2010), Mental Capacity Act (2005) and the Human Rights Act (1998). Above all, you will be committed to improving outcomes for Wirral residents and enhancing the way we engage with them, ensuring they are at the heart of everything we do.
