About the Role

This is an exciting opportunity to join Wirral Council as an Advanced Practitioner (Adults), a pivotal role within our Adult Social Care service, leading and championing high‑quality practice across safeguarding, adult protection and complex casework. The focus of the role is to support people who are in receipt of services with assessment and evaluation to ensure they are accessing all appropriate funding streams to meet their assessed needs. The successful applicant will work as part of a team alongside three new Commissioning and Contracts colleagues. The team will work across operational social care delivery to review new applications for funding against national and local criteria, offering best practice advice to assessment and reviewing officers, alongside guidance and professional standards oversight.

The focus of the work includes:

  • Manage and supervise staff, who will identify individuals who may be eligible for continuing health care.
  • Multi-disciplinary working with health professionals, identifying services & support and utilising funding streams in the locality.
  • Supporting casework for existing funded clients, including those approaching review.
  • Creating and monitoring data, with supporting workflow and system developments, to manage applications and oversee cases effectively.
  • Working closely with Legal Services to support correspondence relating to funding applications and decisions.
  • Monitoring and overseeing risk for the authority, identifying mitigation options, and communicating effectively with senior operational management.
  • Working closely with providers across all sectors on best practice in case recording, case risk assessments, positive behaviour management, and the collation of evidence to support applications.
  • Completed strength based holistic assessments.
  • Undertake care act assessments with a person who could be in supported living or a care home setting.
  • Hold a case load of people who could be eligible for Continuing Healthcare (CHC) and complete relevant documentation as required.
  • Complete mental capacity assessments, attend and chair professional meetings when appropriate.
  • Effective time management, plan and coordinate caseload, ensure all documentation is completed in a timely and effective manner.
  • Monitoring service provision and standards in accordance with Registration and Inspection Department.
  • Complete CHC checklists.
  • Attend DST/MDTs with health colleagues.
  • Complete community reviews /placement reviews-ensuring that needs are being met in the appropriate environments.
  • Promoting independence and empower.

About the Candidate

We are looking for an experienced, confident, and motivated practitioner who is passionate about safeguarding adults, promoting independence and improving outcomes for vulnerable people. You will need a sound knowledge and experience of CHC and a comprehensive understanding of the National Framework for NHS Continuing Healthcare and NHS-Funded Nursing Care 2022. You will also bring a constructive, forward‑thinking approach - embracing change, promoting high standards and helping shape a positive and inclusive team culture. This role combines expert practice with leadership responsibilities, making it an ideal position for an experienced social worker who is ready to deepen their impact and shape professional standards across the service.
