About the Role: Wirral Council is looking for an experienced and motivated Practice Lead Reablement to join our Community Integrated Response Team (CIRT). This is an exciting leadership opportunity to help shape and deliver high-quality reablement, early intervention and discharge services that promote independence and wellbeing for Wirral residents. Working within a multidisciplinary health and social care environment, you will provide leadership, management and professional support to frontline staff, ensuring safe, effective and person-centred practice. You will oversee complex casework, support hospital discharge pathways, drive service improvement, and work closely with health colleagues and partner organisations to deliver positive outcomes for individuals and carers. As a key member of the leadership team, you will champion strengths-based approaches, quality assurance, continuous improvement and integrated working across the service.
About the Candidate: We are looking for a confident and compassionate leader with significant experience in health and/or social care services. You will have a relevant Health and Social Care qualification (NVQ Level 3 or equivalent) and management qualification, alongside a strong understanding of legislation including the Care Act, Safeguarding and Mental Capacity Act. The successful candidate will have experience of leading teams, managing performance, supporting professional development and working collaboratively across organisational boundaries. You will be an excellent communicator with the ability to make sound decisions, manage competing priorities and inspire others to deliver outstanding person-centred care. Most importantly, you will share Wirral Council's commitment to helping people maximise their independence, achieve their goals and live well within their communities.
