About the Role: As a Placement & Brokerage Officer, you’ll play a vital part in our Care Arrangement Team, coordinating timely and effective care packages for adults with assessed needs. You will be instrumental in supporting safe hospital discharges, brokering care that enables people to return home with the right support, and working closely with care providers to match individuals to services that meet their needs.

About the Candidate: Organised, proactive, and confident, you communicate well with professionals, providers, and stakeholders alike. You have strong IT skills, a keen eye for detail, and the ability to manage competing priorities with calm and professionalism. You work well in a team, are committed to high standards, and care deeply about delivering safe, person-centred outcomes.
