An exciting opportunity has arisen for a Social Worker to join the Transfer of Care Hub Adult Social Care Team within Wirral University Teaching Hospitals. This role is primarily located at Arrowe Park Hospital, with additional in‑reach support to Clatterbridge Hospital.

About the Role

You will work as part of a supportive, specialist team focused on delivering high‑quality support to aid in safe and timely discharge from hospital for the residents of Wirral. The service operates 7 days a week; weekends and bank holidays are worked on a rota basis. This role offers an opportunity to broaden your skills and gain experience in a different service area.

About the Candidate

We are looking for a Social Worker, preferably with experience in hospital discharge, but this is not essential. All applicants must be registered with Social Work England or be in the process of registering. Experience with discharge planning and working with multidisciplinary teams is valued.

This post is subject to an enhanced DBS check, including a check of the relevant Barred List(s). Eligibility to work in the UK will be verified during the final stages of the selection process. All candidates must either already have a right to work in the UK before commencing employment or meet the Home Office criteria for visa sponsorship. Candidates who require sponsorship should evaluate the Home Office eligibility criteria for a Skilled Worker visa before applying.
