The role will be offered on a 6‑month fixed‑term basis, with the possibility of being extended for up to 2 years.

About The Role

  • Based across Arrowe Park Hospital and Clatterbridge offices, you will work within a dynamic multi‑disciplinary team, supporting safe, timely discharges from hospital while promoting independence, wellbeing, and positive outcomes.
  • Contribute to the delivery of high‑quality health and social care functions for adults and carers, ensuring the practice reflects the Care Act 2014, strength‑based working, personalisation, and social inclusion.
  • Work with adults with physical health needs, mental health needs, older adults, and carers—often those experiencing complex circumstances.
  • Support the development of others, including ASYE/newly qualified social workers and Care Navigators, modelling good practice and championing professional standards.

About The Candidate

  • Strong background working with adults with complex needs, carers and families.
  • Skilled in assessments, support planning, safeguarding, and working within statutory frameworks.
  • Excellent written and verbal communication skills, with the ability to manage complex and sensitive situations, prioritise demanding workloads, and maintain high‑quality practice.
  • Social Work qualification required (e.g. Degree in Social Work, DipSW or equivalent) and registered with Social Work England.
