About The Role
The Social Care Worker will support the delivery of services for adults and carers, promoting personalisation and inclusion while ensuring practice aligns with the Care Act 2014 and strength‑based approaches. Key duties include:
- assessing needs,
- coordinating and implementing appropriate social care services,
- providing advice, guidance, and support to adults with complex physical and mental health needs, as well as older people and carers.
