An exciting opportunity has arisen within CIRT UCR (Crisis Intervention & Rapid Response / Urgent Community Response) as we look to appoint a committed and forward‑thinking Advanced Practitioner to join our team. UCR plays a vital role in supporting people within our community who are experiencing crisis. The service delivers rapid assessment and timely intervention within the national 2‑hour response standard, helping to prevent avoidable hospital admissions and ensuring individuals receive high‑quality, person‑centred care in the most appropriate setting.

As an Advanced Practitioner, you will be central to maintaining and further strengthening this offer—bringing clinical expertise, strong leadership, and professional curiosity to a fast‑paced and rewarding environment:

  • Leading on complex assessments and high‑risk cases, ensuring safe, robust, and person‑centred decision‑making.
  • Providing expert guidance, mentoring, and professional support to practitioners across the service.
  • Championing best practice in safeguarding, risk management, and urgent community care.
  • Working collaboratively with multi‑agency partners to deliver integrated, effective crisis responses.
  • Contributing to service development, innovation, and continuous improvement across UCR.

We are seeking a highly skilled, motivated practitioner with substantial post‑qualifying experience, excellent assessment and decision‑making abilities, and a passion for delivering high‑quality urgent care. You will bring strong leadership qualities, the ability to influence practice, and a commitment to driving positive outcomes for individuals and carers.
