An exciting opportunity has arisen at our award-winning practice for a Healthcare Assistant & Care Coordinator to join our fast-growing, friendly team. This is a dual role, combining hands-on clinical support with care coordination responsibilities. You will work closely with our multidisciplinary team to support patient care, service delivery, and smooth day-to-day practice operations. Full training and development will be provided where required.

Our practice provides a wide range of services, including:

  • AAA screening services
  • Midwifery

A patient population of 5,500. Training and research opportunities. Our team includes: 2 GP Partners, 1 Salaried GP, Clinical Pharmacists, 2 Nurses, Healthcare Assistant, GP Assistants, Care Coordinators, and Social Prescribers. A well-organised reception and administrative team. We use modern technology to support patient care, including EMIS Web, AccuRx, and Docman 10, ensuring smooth and efficient service delivery.

At Winshill Medical Centre, our Care Coordinators play a key role in supporting patients, including those in care homes, and helping our multidisciplinary team deliver seamless, patient-centered care.

Job responsibilities

Healthcare Assistant Core Responsibilities

The Healthcare Assistant will primarily support clinical and administrative services within the practice. While the role has defined core responsibilities, occasional additional duties may be required depending on workload and staffing levels.

Clinical Support:

  • Support the practice nurse in health promotion programmes.
  • Perform baseline observations, including blood pressure, pulse, temperature, and oxygen saturation, recording findings accurately.
  • Undertake wound care, dressings, and other clinical tasks as required.
  • Assist the practice nurse in the management of chronic disease clinics.
  • Act as a chaperone when required.
  • Perform venepuncture.
  • Conduct ECGs as requested.
  • Ensure specimens are collected, recorded, and prepared for onward transportation.
  • Provide support during minor operations as needed.
  • Ensure clinical rooms are fully stocked and prepared for each session.
  • Maintain fridge hygiene in accordance with current guidance.
  • Ensure clinical waste and sharps are managed according to the organisations IPC policy.
  • Deliver opportunistic health promotion where appropriate.

Care Coordination Support:

  • Support the Care Coordination Team during weekly care home ward rounds.
  • Assist with administrative duties related to care home patients, reporting to the lead care home GP.

Secondary Responsibilities

In addition to the core duties, the Healthcare Assistant may be requested to:

  • Participate in practice audits as directed by the audit lead.
  • Take part in local initiatives to enhance service delivery and patient care.
  • Support and participate in shared learning within the organisation.
  • Complete opening and closing procedures in accordance with the duty rota.

Person Specification

Qualifications

  • Healthcare qualification (Level 3 or 4) or currently working towards an equivalent qualification.
  • Experience of working with the general public.
  • Experience of working in a healthcare setting.
  • Competence in ECGs.
  • Competence in venepuncture, including recording height, weight, BP, and pulse.
  • Ability to perform chaperone duties.
  • Ability to record accurate clinical notes.
  • Excellent communication skills (written and verbal).
  • Clear, polite telephone manner.
  • Completion of the HCA Care Certificate (as detailed in GP Mythbuster No. 57 HCAs in General Practice).
  • Experience working in a primary care environment.
  • Experience in wound care, including removal of sutures and staples.
