Our Wiltshire Support at Home Support Workers provide vital assistance to individuals in their homes post-hospital discharge. They play a pivotal role in helping customers achieve their goals, working closely with dedicated occupational therapists to co-develop personalised reablement plans.

As a First Responder Support Worker, you will support with hospital discharges, be the first point of contact for new customers, assess their needs, compile a support plan, and attend to needs such as personal care, food preparation, and daily living activities as required. To meet the needs of the service, shifts include early morning, evenings, weekends, and bank holidays, travelling across the Wiltshire area to carry out customer visits.

We are seeking individuals with a genuine passion for helping others, an understanding of customer care needs, and the ability to compile comprehensive support plans.
