Join our Discharge Service as an Advanced Practitioner and play a key part in supporting safe, timely hospital discharges while leading and developing social care practice across our teams.

About the role

As an Advanced Practitioner, you will provide professional leadership, practice expertise and supervision to Social Workers and Social Care Practitioners within the Hospital Team. You will support the delivery of high-quality, person-centred social work practice and help shape the development of our services. You will also manage a small caseload of complex work, ensuring that assessments, support plans and reviews are completed in line with statutory responsibilities, including the Care Act 2014, Mental Capacity Act 2005 and Mental Health Act 1983. This role plays a crucial part in strengthening partnership working with acute hospitals, community health services and voluntary sector organisations to support safe and effective hospital discharge pathways.

What you’ll be doing

  • Providing professional leadership, guidance and supervision to Social Workers and Social Care Practitioners.
  • Planning, prioritising and allocating work in collaboration with the Team Manager.
  • Leading or participating in multidisciplinary discharge meetings, Best Interest decision-making and cross-agency forums.
  • Managing a small caseload involving highly complex situations using evidence-based, person-centred practice.
  • Promoting safeguarding best practice and supporting staff with defensible decision-making.
  • Driving improvement in service delivery through quality assurance, training, mentoring and promoting evidence-informed practice.
  • Supporting partnership working to ensure safe, timely and effective hospital discharges.

About you

We are looking for someone who is passionate about high-quality social work practice and has the ability to lead, influence and inspire others. You will need:

  • BA/BSc/MA/MSc in Social Work, DipSW, CQSW, CSS or equivalent recognised by Social Work England.
  • Registration with Social Work England.
  • Post-qualifying award (ASYE/PQ) and evidence of ongoing professional development.
  • Significant post-qualification experience in adult social care, ideally within hospital or discharge services.
  • Strong knowledge of social care and NHS legislation, safeguarding procedures and performance frameworks.
  • Experience supervising or developing staff.
  • Excellent assessment, communication, negotiation and report-writing skills.
  • Ability to work proactively, manage complex situations and problem-solve effectively.

Desirable:

  • Best Interests Assessor qualification
  • Practice Educator qualification
  • Experience delivering training or formal supervision
