We are looking for an enthusiastic and experienced Team Manager to lead our Discharge Service in the West Locality. The role is based at the Royal United Hospital but covers a range of settings, including rehabilitation beds in community hospital settings. This is part of a broader county-wide discharge service that offers supportive, integrated working.

We provide a network of support, guidance, and professional development to support you in this role. This role is crucial to ensuring efficient and effective service delivery for adults in Wiltshire, supporting the flow across acute and community settings to achieve positive outcomes for our customers. This includes ensuring “HomeFirst” principles are at the centre of our practice and ensuring assessments/support delivery is provided in a safe and timely manner.

You will play a key role in promoting integrated working with Acute hospitals, Banes, Swindon, Wiltshire Integrated Care Board (BSW ICB), Reablement services, and community partners within the specified locality. As the Team Manager, you will drive good performance management and implement service evaluation systems to maintain high-quality services. You will ensure that Health and Safety compliance, including risk assessments and lone working protocols, are adhered to.

You will support your team through supervision, appraisal, and governance arrangements, ensuring compliance with legislative and regulatory frameworks. You will collaborate with the Head of Service regarding service development and implement change programmes as required.

We are looking for someone with a relevant professional qualification in social care or health, such as a Degree/Diploma in Social Work or Occupational Therapy. A management qualification to level 5 or equivalent experience is essential, along with at least 2 years of management/supervisory experience.
