About the role
In these roles, you will provide professional support, train team members, and act as an expert advisor on equipment and decision-making. You will conduct strengths-based assessments and design innovative reablement plans tailored to individual needs, encouraging independence and minimising long-term care needs. With the autonomy to manage your time and schedule, you will be able to provide bespoke care to individuals in your diverse caseload, ensuring their unique needs are met with compassion and expertise. You will be conducting client visits in their homes or other settings, such as hospitals.
