The Reviewing Team supports adults with long-term and complex needs, helping them live as independently as possible. You will be part of a team that puts people at the heart of everything we do, working closely with individuals, families, and communities to build strong relationships and promote wellbeing.
As a Social Care Practitioner, you will carry out assessments and reviews under the Care Act 2014, coordinate support journeys, and work in partnership with other professionals to create personalised plans that meet individual outcomes. You will be confident working independently, know when to seek advice, and be committed to safeguarding vulnerable adults. This is a fast-paced role and you will be expected to complete assessments and reviews both in person and by telephone.
To succeed in this role, you will need experience in health or social care, strong communication skills, and a good understanding of relevant legislation. You will either hold a Level 4 qualification in Health and Social Care or be willing to work towards one. This is a county-wide role and you will be expected to travel to all areas of Wiltshire, however, we are currently seeking to cover the south of the county predominantly.
