Working across community and hospice settings, you'll deliver expert clinical care while providing emotional support to patients and their families, helping them remain as comfortable and independent as possible.

Your Role:
- Deliver high-quality palliative and end-of-life nursing care to patients in community and hospice settings.
- Assess patients' physical, emotional, and psychological needs, developing and reviewing individualised care plans.
- Safely manage medications and symptom control, including the use of syringe drivers.
- Respond to telephone enquiries, providing appropriate clinical advice and support to patients, families, and healthcare professionals.
- Work closely with the wider multidisciplinary team to coordinate seamless, high-quality care.
- Support patients and their families with compassion, dignity, and clear communication throughout their care journey.
- Maintain accurate clinical records and ensure compliance with NMC standards and organisational policies.
- Contribute to maintaining the highest standards of clinical care and continuous service improvement.
