Job Description
This is a rewarding opportunity for an experienced Registered General Nurse (RGN) to join a dedicated Community Palliative Care Team, supporting patients with complex palliative and end-of-life care needs. Working across community and hospice settings, you'll deliver expert clinical care while providing emotional support to patients and their families, helping them remain as comfortable and independent as possible.,
Key Responsibilities
- Assess patients' physical, emotional, and psychological needs, developing and reviewing individualised care plans.
- Safely manage medications and symptom control, including the use of syringe drivers.
- Respond to telephone enquiries, providing appropriate clinical advice and support to patients, families, and healthcare professionals.
- Work closely with the wider multidisciplinary team to coordinate seamless, high-quality care.
- Support patients and their families with compassion, dignity, and clear communication throughout their care journey.
- Maintain accurate clinical records and ensure compliance with NMC standards and organisational policies.
- Contribute to maintaining the highest standards of clinical care and continuous service improvement.
Essential Conditions
All appointments are subject to satisfactory references, an enhanced DBS check, verification of the right to work in the UK, and any other pre-employment checks required for the role.
Salary and Working Conditions
- Up to Β£51,000 per year (pro rata for part-time, Depending on experience and Non-Medical Prescribing qualification)
- Must be a driver, due to the nature of the role and travel requirements
