We are looking for a Pulmonary Rehabilitation Practitioner to join us, working 22.5 hours per week.
The Southampton COPD team works together with University Hospital Southampton to provide an integrated approach to COPD management for the population of Southampton.
We provide a Pulmonary Rehabilitation PR Pathway & Community COPD clinics for people with complex COPD.
The service is runs at Bitterne Health Centre, Royal South Hants Hospital and Adelaide Health Centre plus community sites.
We are looking for an enthusiastic practitioner who will actively promote the COPD pathway & primarily support the delivery of pulmonary rehabilitation in the community.
They will independently manage & deliver evidence based PR exercise programmes to patients in different community settings, including digital programmes.
The post holder will also support community respiratory clinics across the city supporting consultants & specialist practitioners in delivering complex COPD care.
The successful candidate will work as an active member of the team in supporting & leading patients during their time on our programme.
They will have in depth knowledge of exercise & its prescription, being able to lead & progress patients through their programme.
With this knowledge they will also be a resource for training of other team members & the development of the service.
