An exciting opportunity has arisen for an enthusiastic and motivated Rehabilitation Assistant to join the Neuro Early Supported Discharge Team. The successful candidate would join our team from 3rd August 2026. The Neuro ESD Team is a multidisciplinary team providing specialist intensive rehab to clients with acute stroke and other neurological conditions on discharge from hospital or as community admission avoidance.
The post holder will be working alongside a team of Physio's, Occupational Therapists, Speech & Language Therapists, Clinical Neuropsychologist, Rehab Assistants & Admin, working in the Southampton and West Hampshire areas. The team provides six-week intensive neurorehabilitation to individuals with a stroke or neurological diagnosis, who have been newly discharged from hospital or are at risk of admission into hospital. Care is delivered in a client's own home or alternative community setting.
Working Pattern: Monday - Friday with the expectation for some weekend working.
A full and valid UK driving licence and access to a vehicle for work purposes is required for this role.
