Southampton East Older Persons Community Mental Health Team are on the lookout for a committed and qualified professional to enhance their team based at the Moorgreen Hospital in West End, Southampton.

The Band 6 position presents a thrilling chance to conduct mental health assessments and offer advice and support to our clients. Prior experience with older persons and individuals with severe mental illnesses is preferred, whether in a hospital or community setting. Expertise in Dementia care and assessment would also be an asset for the available position.

The role of a care coordinator is both challenging and fulfilling, serving as the initial point of contact for the elderly facing mental health challenges and significantly influencing the care we deliver in our community.
