We are excited to announce a new opportunity within our Hampshire and Isle of Wight All Age Continuing Care Personalisation service.

We are looking for a person who is passionate about client care and improving life opportunities who share our behaviours of being person centred, honest, courageous, innovative, and collaborative.

Responsibilities

  • As a Personalisation and Personal Health Budget Practitioner, you will be responsible for maximising the take-up of Personal Health Budgets and acting as a source of expertise and advice for relevant members of the multi-disciplinary teams, patients and members of the public in order to support the Personal Health Budget Process.
  • The role involves working in partnership with clinicians to implement Personal Health Budgets, including supporting and advising on the development of care and support plans, monitoring and managing client financial information and payments and providing advice and guidance to clients to empower people with complex health needs live their best lives.
  • Whilst experience in Personal Health Budgets and accounting would be helpful, if you have the right skills and attitude, we can support you to develop the specific skills required.
  • You will be part of a wider personalisation and All Age team who provide a wide range of expertise and support.

Qualifications

  • Experience in Personal Health Budgets and accounting would be helpful, but the right skills and attitude can be developed with support.
  • Ability to work in partnership with clinicians to implement Personal Health Budgets and advise on the development of care and support plans.
