We are looking for aNursing Associate/Associate Practitioner!

You will play a key role in supporting people living with diabetes to manage their condition and achieve their health goals. Working within agreed competencies and under the guidance of registered practitioners, you will contribute to the assessment, planning, delivery, and evaluation of care.

The role may involve visiting patients in their own homes or care homes and includes periods of lone working, requiring confidence, initiative, and excellent communication skills.

We are seeking an enthusiastic individual with excellent interpersonal skills and a genuine commitment to improving the lives of people living with diabetes. You will be able to work independently when required while remaining an effective and supportive member of the wider team.
