HIOWH NHS Trust have an exciting opportunity to join our team. You must be a qualified & Registered Mental Health Nurse, Social Worker, or Occupational Therapist. This role will focus on working within the community and supporting those service users in Portsmouth with mental health conditions and substance misuse.

You will be expected to be co-located with substance misuse services and other organisations within this area of expertise as well as being an integrated part of Mental Health services. The Mental Health Outreach team is a small multidisciplinary team working predominately 9-5 Monday to Friday with some flexibility as required.

The main aim is to provide timely and effective responses for people with co-occurring conditions. This will be achieved by the team working together to improve access to services, which can reduce harm, improve health and enhance recovery, responding effectively and flexibly to presenting needs and prevent exclusion.

The team will be an access point for Adult Mental Health by assessing and working with our service users to access the most appropriate services which are tailored to support their mental health needs. Our service users will be supported by the right professional at the right time.

As a trust we have multiple opportunities for training and development, including leadership training and opportunities for other advanced courses to progress your career. Please note this role is for 28 hours a week and 6 month fixed term.
