Are you a passionate and enthusiastic Healthcare Support Worker? Do you want to be part of a forward thinking, dynamic workforce? If you have answered Yes, then HIOW Healthcare NHS Foundation Trust -- Community Rehabilitation Service (CRS) has an exciting opportunity for you to join our dynamic team. This permanent position is well suited to an individual that is looking to provide excellent nursing care to the Portsmouth Community in a thriving and supportive workplace.

CRS is a multi-disciplinary team made from staff employed by Health and Adult Social Care for residents of the Portsmouth City area. The service aim is to help individuals remain at home and to avoid unnecessary admission to hospital, and to facilitate discharge from hospital by offering assessment of immediate needs. The team work hard to ensure high quality person-centred care is at the heart of everything we do, and our staff are supported in their roles and encouraged to develop. You will be required to work within the wider multi-disciplinary team providing evaluating and communicating effectively with other teams within the Service.

Responsibilities

  • Provide excellent nursing care to residents in the Portsmouth Community as part of the Community Rehabilitation Service.
  • Work within a wider multi-disciplinary team and collaborate with other teams within the Service.
  • Assist in assessment of immediate needs and support discharge planning to facilitate timely hospital discharge or avoidance of unnecessary admissions.
  • Communicate effectively with team members and stakeholders to ensure high quality person-centred care.
