Portsmouth Integrated Learning disability service are seeking to recruit a Learning Disability Nurse to join a busy and well‑established multi-disciplinary CQC rated 'Outstanding' community Learning Disability Service, supporting the health care needs of people with a learning disability in Portsmouth.
You will play a vital role as part of the community learning disability team, within a forward thinking and innovative team. You will receive regular support and supervision and opportunities for professional development.
Your role will be to provide specialist health care services to adults with a learning disability.
To meet the needs of patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment. This is achieved through a process of Person-Centred Care, which is comprehensive and responsive to the needs of the individual and their family/carers. Identified needs will be assessed, treatments implemented and evaluated in partnership with Service Users as well as family/carers.
