Overview
Our teams across Portsmouth are looking for a Community Healthcare Support Worker to work within the multi-disciplinary Integrated Care Team that delivers community based services to a GP Practice population, undertaking nursing tasks within agreed parameters, set out by the registered practitioners. We have roles in Waterlooville, Gosport/Fareham and within our end of life team based at St Mary's campus. Please record your preference within your application.
Responsibilities
- Provide home visiting and lone working without direct supervision.
- Work within service hours of 8am to 8pm, with a requirement to work 1 in 4 weekends.
- Support admission avoidance, early supported discharge and promotion of independence for patients.
- Assist in the effective management of patients with long term conditions and older people with mental health problems.
Qualifications and Requirements
- Full UK driving licence and access to a car is essential.
- Right to work in the UK at the time of application.
