An exciting opportunity has arisen for an enthusiastic and motivated Senior Rehabilitation Assistant to join the Neuro Early Supported Discharge Team. The Neuro ESD Team is a multidisciplinary team providing specialist intensive rehab to clients with acute stroke and other neurological conditions on discharge from hospital or as community admission avoidance.

The post holder will be working alongside a team of Physios, Occupational Therapists, Speech & Language Therapists, Clinical Neuropsychologist, Rehab Assistants & Admin, working in the Southampton and West Hampshire areas. The team provides six-week intensive neurorehabilitation to individuals with a stroke or neurological diagnosis, who have been newly discharged from hospital or are at risk of admission into hospital. Care is delivered in a client’s own home or alternative community setting.

Working Pattern: Monday - Friday with the expectation for some weekend working.

Eligibility: Only open to applicants from Hampshire & Isle of Wight NHS system, which is limited to: Hampshire and Isle of Wight ICB, Hampshire & Isle of Wight Healthcare FT, Portsmouth Hospitals University NHS Trust, University Hospital Southampton FT, South Central Ambulance Service, Isle of Wight NHS FT or Hampshire Hospitals FT.
