We are looking for an enthusiastic and experienced clinician to join our Neuro Early Supported Discharge team within Community Neuro Rehab Services in Southampton as a Clinical Team Lead. The Neuro ESD Team provides intensive neuro-rehab to individuals with a Stroke or Neurological diagnosis, who have been newly discharged from hospital or are at risk of admission into hospital and have a GP within Southampton City or parts of West Hampshire.
The multidisciplinary team comprises of Physiotherapy, Occupational Therapy, Speech & Language Therapy, Nursing, Clinical Psychology, Rehab Assistants and admin, which is delivered in a client's own home or alternative community setting. Intervention from the Early Supported Discharge Teams aims to facilitate improved independence and quality of life based upon agreed personalised rehabilitation goals.
The post holder will have post graduate experience which should include some specific neurological clinical experience, excellent leadership skills and evidence of working within a multidisciplinary team. Experience of service improvement projects or audits is also desirable.
Only open to applicants from Hampshire & Isle of Wight NHS system, which is limited to: Hampshire and Isle of Wight ICB, Hampshire & Isle of Wight Healthcare FT, Portsmouth Hospitals University NHS Trust, University Hospital Southampton FT, South Central Ambulance Service, Isle of Wight NHS FT or Hampshire Hospitals FT.
