Join a new innovative community health team delivering proactive, person-centred care across Southampton neighbourhoods. As a Neighbourhood Care Practitioner, you will work at the intersection of primary care, social support, and community services to prevent hospital admissions, support independent living, and improve wellbeing for people in their homes and local settings. This is a hands-on, autonomous role with strong clinical collaboration and opportunities to shape service design.
Posts available: Three full-time posts, one in each of the three localities across Southampton with cross cover when applicable.
Contract: Fixed 12-month contract with the possibility of extension following evaluation. Hours are full-time, flexible working patterns considered. Shift Type Monday to Friday within core hours.
