We are seeking an enthusiastic and highly-motivated Team Lead to join our Dementia Outreach Team and become part of a service that really makes a difference to patients experiencing a dementia-related crisis, and their families.
As the Team Lead, you will be responsible for providing clinical and managerial leadership with mentoring and educational input to the Dementia Outreach Team multidisciplinary team. This includes acting as a clinical leader and expert practitioner liaising, guiding and advising the multidisciplinary team in the provision of optimum patient experience for people accessing support and assessment.
You will also be responsible for supporting the effective role modelling, ongoing management and clinical supervision, and mentorship to the team. You will be responsible for ensuring that the highest quality care is carried out in accordance with policies and procedures of Hampshire and Isle of Wight NHS Foundation Trust.
You will ideally have experience of working with people with dementia and one of the professional registrations outlined within the attached Job Description and Person Specification. The ability to work in partnership with carers/family members and services in the community is essential.
