Are you an experienced Specialist Nurse and interested in working in the Heart Failure Team, we have 2 fixed term posts within the team and would encourage you to apply if you would like to work in this team in the following way.

To join the existing highly developed integrated, nurse led heart failure service.

To ensure that the service standards meet the requirements of the National Institute of Clinical Excellence guidelines CG187 and NG106.

To provide evidence-based specialist nursing assessment, advice to patients with a confirmed diagnosis of Heart failure with either a preserved ejection fraction or a reduced ejection fraction and support to nurses in the acute or community setting.

To facilitate Primary Health Care Teams and local hospice teams to develop effective and appropriate heart failure pathways.

To work closely with St.

Marys colleagues, CCG colleagues, General Practices, local hospice and Tertiary centres in providing and developing a service that will meet and maintain standards across primary/secondary care pathway.
