We have an exciting opportunity to join us as a Health and Wellbeing Coach. Gosport Central Primary Care Network are looking for a person that can motivate patients to manage their own health & well-being to live independently and improve their health outcomes.

The post holder will work with individuals and/or groups of patients to deliver sessions on how to motivate patients to improve their physical activity, eat healthily to meet personalised care plans. You will use a holistic approach to promote lifestyle changes, support patients to address their existing health problems and prevent new ones.

The role works within their practices multi-disciplinary team, a full induction and training is given to help candidates integrate into Primary Care.
