An exciting opportunity has arisen for a Social Prescriber - Children & Young People to join our Personalised Care Team in Gosport. This rewarding role focuses on supporting children, young people and families, particularly those with Learning Disabilities (LD) and/or Special Educational Needs and Disabilities (SEND), to improve their health and wellbeing through personalised, non-clinical support.
Working within a Primary Care Network (PCN), you will help families identify what matters most to them, navigate local services, access community support, and build confidence and resilience. You will work collaboratively with GP practices, schools, Family Hubs, Children's Services and voluntary sector organisations to ensure families receive timely, coordinated and inclusive support.
The successful candidate will be passionate about improving outcomes for children and families, have excellent communication skills and be able to build strong relationships across a range of services and communities.
You will be working within a team supporting a population of 85,000 patients in Gosport that are registered with Bridgemary Medical Centre, Blossom Health, Rowner Medical Centre, Solent View Medical Practice and The Willow Group GP Surgeries.
