Hampshire and Isle of Wight Healthcare NHS Foundation Trust has an exciting opportunity for you! We are looking for dedicated individuals to join our thriving Community Nursing Service in Portsmouth, Fareham and Southampton in a permanent role, delivering outstanding care to the community in a supportive and collaborative environment.

Our Community Nursing Service provides high-quality nursing care to patients in their own homes, operating 12 hours a day (8am–8pm), 7 days a week, 365 days a year. We pride ourselves on delivering person-centred care and ensuring our staff feel valued, supported, and encouraged to develop their skills.

In this role, you will work as part of a wider multi-disciplinary team, contributing to care planning, evaluation, and effective communication across services. A full driving license and access to a vehicle is required for this role. There is a space later in the application to put your preferred location.
