Gosport Central PCN is seeking a qualified clinical pharmacist on a fixed term contract until 31 March 2028 to deliver structured medication reviews for the 85,500 residents of Gosport. This role supports Integrated Neighbourhood Working to reduce hospital admissions and promote independence within the community.
You will join a multidisciplinary team including a Nurse, Pharmacy Technician, OT, Physiotherapist, Paramedic and GP Assistant to help deliver the scheme's aims.
